POST PARTUM

A. PENGERTIAN POST PARTUM
Persalinan adalah suatu proses pengeluaran hasil konsepsi yang dapat hidup dari dalam uterus melalui vagina kedunia luar. Persalinan imatur adalah persalinan saat kehamilan 20-28 minggu dengan berat janin antara 500-1000gr. Persalinan premature adalah persalinan saat kehamilan 29-36 minggu dengan berat janin antara 1000-2500 gr ( Barbara F. weller 2005 ).
Persalinan normal adalah proses kelahiran bayi dengan tenaga ibu sendiri tanpa bantuan alat-alat serta tidak melukai ibu dan bayi. Persalinan adalah proses pengeluaran hasil konsepsi (janin dan ari) yang dapat hidup ke dunia luar dan  rahim melalui jalan lahir atau dengan jalan lain (Mitayani, 2009).
Pada saat persalinan ada 3 faktor yang perlu diperhatikan, yaitu:
1. jalan lahir (tulang dan jaringan lunak pada panggul ibu)
2. janin dan kekuatan ibu
3. Kelainan satu atau beberapa faktor diatas dapat menyebabkan distosia.

B. ETIOLOGI
Penyebab persalinan belum pasti diketahui,namun beberapa teori menghubungkan dengan faktor hormonal,struktur rahim,sirkulasi rahim,pengaruh tekanan pada saraf dan nutrisi (Hafifah, 2011)
1. Teori penurunan hormone
1-2 minggu sebelum partus mulai, terjadi penurunan hormone progesterone dan estrogen. Fungsi progesterone sebagai penenang otot –otot polos rahim dan akan menyebabkan kekejangan pembuluh darah sehingga timbul his bila progesterone turun.
2. Teori placenta menjadi tua
Turunnya kadar hormone estrogen dan progesterone menyebabkan kekejangan pembuluh darah yang menimbulkan kontraksi rahim.
3. Teori distensi rahim
Rahim yang menjadi besar dan merenggang menyebabkan iskemik otot-otot rahim sehingga mengganggu sirkulasi utero-plasenta.
4. Teori iritasi mekanik
Di belakang servik terlihat ganglion servikale(fleksus franterrhauss). Bila ganglion ini digeser dan di tekan misalnya oleh kepala janin akan timbul kontraksi uterus.
5. Induksi partus
Dapat pula ditimbulkan dengan jalan gagang laminaria yang dimasukan dalam kanalis servikalis dengan tujuan merangsang pleksus frankenhauser, amniotomi pemecahan ketuban), oksitosin drip yaitu pemberian oksitosin menurut tetesan perinfus.

D. TANDA DAN GEJALA
1. Perubahan fisik
a) Involusi uterus
Adalah proses kembalinya alat kandungan uterus dan jalan lahir setelah bayi dilahirkan sehingga mencapai keadaan seperti sebelum hamil.  Setelah plasenta lahir, uterus merupakan alat yang keras, karena kontraksi ini menyebabkan rasa nyeri/mules-mules yang disebut after pain post partum terjadi pada hari ke – 2-3 hari
b) Kontraksi uterus
Intensistas kontraksi uterus meningkat setelah melahirkan berguna untuk mengurangi volume cairan intra uteri.  Setelah 1 – 2 jam post partum, kontraksi menurun stabil berurutan, kontraksi uterus menjepit pembuluh darah pada uteri sehingga perdarahan setelah plasenta lahir dapat berhenti.
c) After pain
Terjadi karena pengaruh kontraksi uterus, normal sampai hari ke -3.  After pain meningkat karena adanya sisa plasenta pada cavum uteri, dan gumpalan darah (stoll cell) dalam cavum uteri .
d) Endometrium
Pelepasan plasenta dan selaput janin dari dinding rahim terjadi pada stratum spunglosum, bagian atas setelah 2 – 3 hari tampak bahwa lapisan atas dari stratum sponglosum yang tinggal menjadi nekrosis keluar dari lochia.  Epitelisasi endometrium siap dalam 10 hari, dan setelah 8 minggu endometrium tumbuh kembali.
Epitelisasi tempat plasenta + 3 minggu tidak menimbulkan jaringan parut, tetapi endometrium baru, tumbuh di bawah permukaan dari pinggir luka.
e) Ovarium
Selama hamil tidak terjadi pematangan sel telur.  Masa nifa terjadi pematangan sel telur, ovulasi tidak dibuahi terjadi mentruasi, ibu menyusui mentruasinya terlambat karena pengaruh hormon prolaktin.
f) Lochia
Adalah cairan yang dikeluarkan dari uterus melalui vagina dalam masa nifas, sifat lochia alkalis sehingga memudahkan kuman penyakit berkembang biak.  Jumlah lebih banyak dari pengeluaran darah dan lendir waktu menstruasi, berbau anyir, tetapi tidak busuk.
g) Lochia dibagi dalam beberapa jenis :
1) Lochia rubra
Pada hari 1 – 2 berwarna merah, berisi lapisan decidua, sisa-sisa chorion, liguor amni, rambut lanugo, verniks caseosa sel darah merah.
2) Lochia sanguinolenta
Dikeluarkan hari ke 3 – 7 warna merah kecoklatan bercampur lendir, banyak serum selaput lendir, leukosit, dan kuman penyakit yang mati.
3) Lochia serosa
Dikeluarkan hari ke 7 – 10, setelah satu minggu berwarna agak kuning cair dan tidak berdarah lagi.
4) Lochia alba
Setelah 2 minggu, berwarna putih jernih, berisi selaput lendir, mengandung leukosit, sel epitel, mukosa serviks dan kuman penyakit yang telah mati.
h) Serviks dan vagina
Beberapa hari setelah persalinan, osteum externum dapat dilalui oleh 2 jari dan pinggirnya tidak rata (retak-retak).  Pada akhir minggu pertama hanya dapat dilalui oleh 1 jari saja.  Vagina saat persalinan sangat diregang lambat laun mencapai ukuran normal dan tonus otot kembali seperti biasa, pada minggu ke-3 post partum, rugae mulai nampak kembali.
i) Perubahan pada dinding abdomen
Hari pertama post partum dinding perut melipat dan longgar karena diregang begitu lama.  Setelah 2 – 3 minggu dinding perut akan kembali kuat, terdapat striae melipat, dastosis recti abdominalis (pelebaran otot rectus/perut) akibat janin yang terlalu besar atau bayi kembar.
j) Perubahan Sistem kardiovaskuler
Volume darah tergantung pada jumlah kehilangan darah selama partus dan eksresi cairan extra vasculer.Curah jantung/cardiac output kembali normal setelah partus. Fungsi ginjal normal, dinding kandung kemih memperlihatkan oedema dan hiperemi karena desakan pada waktu janin dilahirkan.
k) Perubahan sistem urinaria
Kadang-kadang oedema trigonum, menimbulkan obstruksi dari uretra sehingga terjadi retensio urin.  Pengaruh laserasi/episiotomi yang menyebabkan refleks miksi menurun.
l) Perubahan sistem Gastro Intestina
Terjadi gangguan rangsangan BAB atau konstipasi 2 – 3 hari post partum.  Penyebabnya karena penurunan tonus pencernaan, enema, kekakuan perineum karena episiotomi, laserasi, haemorroid dan takut jahitan lepas.
m) Perubahan pada mammae
Hari pertama bila mammae ditekan sudah mengeluarkan colustrum.  Hari ketiga produksi ASI sudah mulai dan jaringan mammae menjadi tegang, membengkak, lebut, hangat dipermukaan kulit (vasokongesti vaskuler)
n) Laktasi
Pada waktu dua hari pertama nifas keadaan buah dada sama dengan kehamilan.  Buah dada belum mengandung susu melainkan colustrum yang dapat dikeluarkan dengan memijat areola mammae.
o) Colustrum yaitu cairan kuning dengan berat jenis 1.030 – 1,035 reaksi alkalis dan mengandung protein dan garam, juga euglobin yang mengandung antibodi.
p) Temperatur
Temperatur pada post partum dapat mencapai 38 0C dan normal kembali dalam 24 jam.  Kenaikan suhu ini disebabkan karena hilangnya cairan melalui vagina ataupun keringat, dan infeksi yang disebabkan terkontaminasinya vagina.
q) Nadi
Umumnya denyut nadi pada masa nifas turun di bawah normal.  Penurunan ini akibat dari bertambahnya jumlah darah kembali pada sirkulasi seiring lepasnya placenta.  Bertambahnya volume darah menaikkan tekanan darah sebagai mekanisme kompensasi dari jantung dan akan normal pada akhir minggu pertama.
r) Tekanan Darah
Keadaan tensi dengan sistole 140 dan diastole 90 mmHg baik saat kehamilan ataupun post partum merupakan tanda-tanda suatu keadaan yang harus diperhatikan secara serius.
s) Hormon
Hormon kehamilan mulai berkurang dalam urine hampir tidak ada dalam 24 hari, setelah 1 minggu hormon kehamilan juga menurun sedangkan prolaktin meningkat untuk proses laktasi

E. KOMPLIKASI
1. Perdarahan post pastum (keadann kehilangan darah lebih dari 500 mL selama 24 jam pertama sesudah kelahiran bayi)
2. Infeksi
a. Endometritis (pembengkakan/radang edometrium)
b. Miometritis / metritis (pembengkakan/radang otot-otot uterus)
c. Perimetritis (pembengkakan/radang peritoneum disekitar uterus)
d. Caked breast / bendungan asi (payudara mengalami distensi, menjdi keras & berbenjol-benjol)
e. Mastitis (Mamae membesar & nyeri & pada suatu tempat, kulit merah, membengkak sedikit, & nyeri pada perabaan ;  Jika tak ada pengobatan bisa terjadi abses)
f. Trombophlebitis (terbentuknya pembekuan darah dlm vena varicose superficial yg menyebabkan stasis & hiperkoagulasi pada kehamilan & nifas, yg ditandai dgn kemerahan / nyeri.)
g. Luka perineum (Ditandai dgn : nyeri local, disuria, temperatur naik 38,3 °C, nadi < 100x/ menit, edema, peradangan & kemerahan pada tepi, pus / nanah warna kehijauan, luka kecoklatan / lembab, lukanya meluas)
3.    Gangguan psikologis
a.       Depresi post partum
b.      Post partum Blues
c.       Post partum Psikosa
4.    Gangguan involusi uterus

H. DIAGNOSA KEPERAWATAN
1. Nyeri akut berhubungan dengan trauma mekanis, edema / pembesaran jaringan atau distensi efek – efek hormonal
2.  Ketadakefektifan menyusui berhubungan dengan tingkat pengetahuan, pengalaman sebelumnya, tingkat dukungan, karakteristik payudara
3.  Resiko tinggi terhadap infeksi berhubungan dengan trauma jaringan, penurunan Hb, prosedur invasive, pecah ketuban, malnutrisi
4.   Perubahan eliminasi urine berhubungan dengan efek hormonal, trauma mekanis, edema jaringan, efek anastesi ditandai dengan distensi kandung kemih, perubahan – perubahan jumlah / frekuensi berkemih
5.  Resiko tinggi terhadap kekurangan volume cairan berhubungan dengan penurunan masukan / penggantian tidak adekuat, kehilangan cairan berlebih (muntah, hemoragi, peningkatan keluaran urine)
6.   Konstipasi berhubungan dengan penurunan tonus otot, efek progesteron, dehidrasi, nyeri perineal ditandai dengan perubahan bising usus, feses kurang dari biasanya
7.   Kurang pengetahuan (kebutuhan belajar) mengenai perawatan diri dan bayi berhubungan dengan kurang pemahaman, salah interpretasi tidak tahu sumber – sumber
8.   Keterbatasan gerak dan aktivitas berhubungan dengan nyeri luka jahitan perineum

I. PERENCANAAN
1. Nyeri akut berhubungan dengan trauma mekanis, edema / pembesaran jaringan atau distensi efek – efk hormonal.

Tujuan dan Kreteria Evaluasi:
Setelah diberikan asuhan keperawatan diharapkan nyeri ibu berkurang dengan criteria evaluasi: skala nyeri 0-1, ibu mengatakan nyerinya berkurang sampai hilang, tidak merasa nyeri saat mobilisasi, tanda vital dalam batas normal. S = 36-370C. N = 60-80 x/menit, TD = 120/80 mmhg, RR= 18 – 20 x / menit
Intervensi dan Rasional:
a. Kaji ulang skala nyeri
Rasional : mengidentifikasi kebutuhan dan intervensi yang tepat
b. Anjurkan ibu agar menggunakan teknik relaksasi dan distraksi rasa nyeri
Rasional : untuk mengalihkan perhatian ibu dan rasa nyeri yang dirasakan
c. Motivasi : untuk mobilisasi sesuai indikasi
Rasional : memperlancar pengeluaran lochea, mempercepat involusi dan mengurangi       nyeri secara bertahap.
d. Berikan kompres hangat
Rasional : meningkatkan sirkulasi pada perinium
e. Delegasi pemberian analgetik
Rasional : melonggarkan system saraf perifer sehingga rasa nyeri berkurang

2. Ketadakefektifan menyusui berhubungan dengan tingkat pengetahuan, pengalaman sebelumnya, tingkat dukungan, karakteristik payudara.

Tujuan dan Kreteria Evaluasi:
Setelah diberikan asuhan keperawatan diharapkan ibu dapat mencapai kepuasan menyusui dengan criteria evaluasi: ibu mengungkapkan proses situasi menyusui, bayi mendapat ASI yang cukup.
Intervesi dan Rasional:
a. Kaji ulang tingkat pengetahuan dan pengalaman ibu tentang menyusui sebelumnya.
Rasional: membantu dalam mengidentifikasi kebutuhan saat ini agar memberikan  intervensi yang tepat.
b. Demonstransikan dan tinjau ulang teknik menyusui
Rasional: posisi yang tepat biasanya mencegah luka/pecah putting yang dapat merusak dan mengganggu.
c. Anjurkan ibu mengeringkan puting setelah menyusui
Rasional : agar kelembapan pada payudara tetap dalam batas normal.

3. Resiko tinggi terhadap infeksi berhubungan dengan trauma jaringan, penurunan Hb, prosedur invasive, pecah ketuban, malnutrisi
Tujuan dan Kreteria Evaluasi:
Setelah diberikan askep diharapkan infeksi pada ibu tidak terjadi dengan KE : dapat mendemonstrasikan teknik untuk menurunkan resiko infeksi, tidak terdapat tanda-tanda infeksi.
Intervensi dan Rasional:
a. Kaji lochea (warna, bau, jumlah) kontraksi uterus dan kondisi jahitan episiotomi.
Rasional : untuk dapat mendeteksi tanda infeksi lebih dini dan mengintervensi dengan tepat.
b. Sarankan pada ibu agar mengganti pembalut tiap 4 jam.
Rasional : pembalut yang lembab dan banyak darah merupakan media yang menjadi tempat berkembangbiaknya kuman.

c. Pantau tanda-tanda vital.
Rasional : peningkatan suhu > 38°C menandakan infeksi.
d. Lakukan rendam bokong.
Rasional : untuk memperlancar sirkulasi ke perinium dan mengurangi udema.
e. Sarankan ibu membersihkan perineal dari depan ke belakang.
Rasional : membantu mencegah kontaminasi rektal melalui vaginal.

4. Perubahan eliminasi urine berhubungan dengan efek hormonal, trauma mekanis, edema jaringan, efek anastesi ditandai dengan distensi kandung kemih, perubahan – perubahan jumlah / frekuensi berkemih.
Tujuan dan Kreteria Evaluasi:
Setelah diberikan askep diharapkan ibu tidak mengalami gangguan eliminasi (BAK) dengan KE: ibu dapat berkemih sendiri dalam 6-8 jam post partum tidak merasa sakit saat BAK, jumlah urine 1,5-2 liter/hari.
Intervensi dan Rasional:
a. Kaji dan catat cairan masuk dan keluar tiap 24 jam.
Rasional: mengetahui balance cairan pasien sehingga diintervensi dengan tepat.
b. Anjurkan berkamih 6-8 jam post partum.
Rasional: melatih otot-otot perkemihan.
c. Berikan teknik merangsang berkemih seperti rendam duduk, alirkan air keran.
Rasional: agar kencing yang tidak dapat keluar, bisa dikeluarkan sehingga tidak ada retensi.
d. Kolaborasi pemasangan kateter.
Rasional: mengurangi distensi kandung kemih.

5. Resiko tinggi terhadap kekurangan volume cairan berhubungan dengan penurunan masukan/penggantian tidak adekuat, kehilangan cairan berlebih (muntah, hemoragi, peningkatan keluaran urine)
Tujuan dan Kreteria Evaluasi:
Setelah diberikan askep ibu diharapkan tidak kekurangan volume cairan dengan KE : cairan masuk dan keluar seimbang, Hb/Ht dalam batas normal (12,0-16,0 gr/dL)
Intervensi dan Rasional:
a. Ajarkan ibu agar massage sendiri fundus uteri.
Rasional: memberi rangsangan pada uterus agar berkontraksi kuat dan mengontrol perdarahan.
b. Pertahankan cairan peroral 1,5-2 Liter/hari
Rasional: mencegah terjadinya dehidrasi.
c. Observasi perubahan suhu, nadi, tensi.
Rasional: peningkatan suhu dapat memperhebat dehidrasi.
d. Periksa ulang kadar Hb/Ht.
Rasional: penurunan Hb tidak boleh melebihi 2 gram%/100 dL.

6. Konstipasi berhubungan dengan penurunan tonus otot, efek progesteron, dehidrasi, nyeri perineal ditandai dengan perubahan
Tujuan dan Kreteria Evaluasi:
Setelah diberikan askep diharapkan konstipasi tidak terjadi pada ibu dengan KE : ibu dapat BAB maksimal hari ke 3 post partum, feses lembek.
Intervensi dan Rasional:
a. Anjurkan pasien untuk melakukan ambulasi sesuai toleransi dan meningkatkan secara progresif.
Rasional: membantu meningkatkan peristaltik gastrointestinal.
b. Pertahankan diet reguler dengan kudapan diantara makanan, tingkatkan makan buah dan sayuran.
Rasional: makanan seperti buah dan sayuran membantu meningkatkan peristaltik usus.
c. Anjurkan ibu BAB pada WC duduk.
Rasional: mengurangi rasa nyeri.
d. Kolaborasi pemberian laksantia supositoria.
Rasional: untuk mencegah mengedan dan stres perineal.

7.  Kurang pengetahuan (kebutuhan belajar) mengenai perawatan diri dan bayi berhubungan dengan kurang pemahaman, salah interpretasi tidak tahu sumber – sumber
Tujuan dan Kreteria Evaluasi:
Setelah diberikan askep diharapkan pengetahuan ibu tentang perawatan dini dan bayi bertambah dengan KE : mengungkapkan kebutuhan ibu pada masa post partum dan dapat melakukan aktivitas yang perlu dilakukan dan alasannya seperti perawatan bayi, menyusui, perawatan perinium.
Intervensi dan Rasional:
a. Berikan informasi tentang perawatan dini (perawatan perineal) perubahan fisiologi, lochea, perubahan peran, istirahat, KB.
Rasional: membantu mencegah infeksi, mempercepat penyembuhan dan berperan pada adaptasi yang positif dari perubahan fisik dan emosional.
b. Berikan informasi tentang perawatan bayi (perawatan tali pusat, ari, memandikan dan imunisasi).
Rasional: menambah pengetahuan ibu tentang perawatan bayi sehingga bayi tumbuh dengan baik.
c. Sarankan agar mendemonstrasikan apa yang sudah dipelajari.
Rasional : memperjelas pemahaman ibu tentang apa yang sudah dipelajari.

8. Keterbatasan gerak dan aktivitas berhubungan dengan nyeri luka jahitan perineum
Tujuan dan Kreteria Evaluasi:
Setelah diberikan askep diharapkan gerak dan aktivitas terkoordinasi dengan KE : sudah tidak nyeri pada luka jahitan saat duduk, luka jahitan perinium sudah tidak sakit (nyeri berkurang).
Intervensi dan Rasional:
a. Anjurkan mobilisasi dan latihan dini secara bertahan
Rasional : meningkatkan sirkulasi dan aliran darah ke ekstremitas bawah.
b. KIE perawatan luka jahitan periniom.
Rasional : mempercepat kesembuhan luka sehingga memudahkan gerak dan aktivitas.
c. Kolaborasi pemberian analgetik.
Rasional : melonggarkan sistem saraf parifer sehingga rasa nyeri berkurang
